To receive a refund check up to $4.29, please complete the original request form and mail it with:
a) The original store identified, itemized cash register receipt dated between 9/7/10 and 12/31/10 with the purchase price circled, and
b) The original UPC symbol from a package of Hearty Medley product
Purchase the product between 9/7/10 AND 12/31/10. Refund requests must be postmarked by 1/15/11 and received by 1/29/11.
